POSITION TITLE: Daily Living Skills Trainer-Floater (DLSTF)

 

PREREQUISITES:

  1. High School diploma or equivalent.
  2. Desire to work in developmental disability field.
  3. Must be literate (able to read and write) at least on a 10th grade level.
  4. Must be 21 years or older.
  5. Valid Louisiana drivers license.
  6. Acceptable motor vehicle report.
  7. Certified in CPR/First Aid.  (Must be obtained prior to employment).

 

POSITION FUNCTION: The DLST provides skills training to the residents as deemed appropriate by the interdisciplinary team, and assists in maintaining a congenial and homelike atmosphere within the community home and provides assistance to the House Manager, all staff and clients, as needed.

 

SPECIFIC DUTIES:

 

  1. Must be able to provide direct – care services for all individuals in the home.  Provides care during the week in the event of illness of a resident, when necessary.  Also provides assistance in getting medical treatment when necessary.  This may include taking residents to Doctor’s appointments.
  2. Work in various Community Homes as needed.
  3. Provides appropriate role models for clients and other staff members in interpersonal relationships and behaviors.
  4. Be able to provide necessary direct care services for all individuals in the home.
  5. Participates in the development of program plans for the individual residents.
  6. Implements resident’s program plan and document as directed.
  7. Assists in maintaining the home, grounds, and vehicles in a clean, orderly, safe fashion.
  8. Assists in maintaining/repair needs of the home or vehicle.  Report any needed maintenance to the House Manager or Assistant House Manager.
  9. Assists in maintaining records of individual resident’s monies.  To provide receipts, tickets or vouchers to account for funds, as required by the agency.
  10. Assists in maintaining a daily log of events in the home.
  11. Maintains records as directed for medication, significant contacts, data collection, physicians orders, etc.
  12. Assures the safety and health of residents within the home.  Reports all injuries and/or emergencies to the House Manager / office / on-call.
  13. Provides transportation to and from work / school and planned activities when necessary.
  14. Interfaces with consultants, social services department, office personnel, parents, neighbors, and other service agencies, etc., in a congenial professional manner.
  15. Provides personal care, laundry, food preparation and grooming services for individuals not able to accomplish these skills by themselves.  Assist in providing training to the individuals to enhance their daily living skills.
  16. Be available to work rotating schedules, (permanent work schedule), to include weekends with ample notice, (2 weeks required by agency policy).
  17. Be able to lift 35 – 40 lbs. if necessary.
  18. Must attend and complete all training required by the agency.
  19. Must be willing to take the Medication Attendant Certification course.  If asked to become certified, must pass and maintain this certification throughout your employment with The Arc of Caddo-Bossier.
  20. Maintain at a minimal a satisfactory job performance evaluation.  Continuous efforts should be made to strive for excellence.
  21. Assist in promoting harmony, encouraging communication, cooperation and positive participation among residents / co-workers at all times.

SUPERVISORY RESPONSIBILITIES: None

**Personally – owned vehicle use is authorized for this position when approved by Supervisor.
